Pembrokeshire Community has an exciting opportunity for a highly motivated and experienced nurse to join our Community Long Term Conditions Team in Pembrokeshire.
With responsibility for leading on the development of our community respiratory service, you will be an excellent communicator and able to work autonomously. You will also be part of the wider integrated multidisciplinary team, supporting the proactive management and co-ordination of care for an identified caseload of patients with multiple complex long term conditions.
You will be patient focused and have the ability to undertake clinical assessments to arrive at a differential diagnosis, start, plan and evaluate care. You will also have organisational and time management skills and be able to demonstrate leadership skills across the inter-professional team.
You must be self-motivating, organised, computer literate and able to demonstrate excellent interpersonal skills with a commitment to continued personal development. This role would offer opportunities to expand your knowledge and skills in other long term conditions as well as consolidating your existing knowledge.
We would be willing to offer this post as an Annex 21 development opportunity to applicants with less experience/knowledge, subject to existing qualifications and the attached Agenda for Change Annex 21 : ( Arrangements for pay and banding of trainees).
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
