Skegness/Mablethorpe area Base Site – To be agreed.
The Specialist Community Dietitian will provide a specialist, cost-efficient and high quality nutrition and dietetic service to support patients requiring dietetic input in community settings. This support will primarily be nutritional support but will include a range of conditions and the service will be delivered to patients and carers in nursing/care homes, patients’ own homes and other locations, e.g. GP surgeries. Consultations and support will be via telephone, video consultation and home visits as required.
You will work alongside a number of teams in this role. You will be a member of the primary care and community teams in the Skegness and Mablethorpe areas and will also work with the ULHT Dietetic Team to ensure a seamless service. As a member of the Dietetic team you will be supported in your developing skills , we are committed to formal and informal training and development and have regular CPD. Professional support and supervision will be provided.
To lead on the development and evaluation of the post and service and the post will develop as it progresses.
To contribute to evidence-based policy, guidelines and procedures in managing nutritional care in the area.
To reduce the risk of malnutrition in the client group, both by individual consultation and education to carers and staff in community settings.
To support appropriate and cost effective use of Oral Nutritional Supplements (ONS) in the community.
To work with other agencies and healthcare professionals to promote the organisation of effective nutritional support in community settings, primarily care/nursing homes
