About the role:
According to data from the Office of National Statistics (ONS), approximately 7.4 million people live with cardiovascular disease (CVD), almost five million with Type 2 diabetes, and respiratory disease is the third biggest cause of death. Prevention, early detection and intervention of these long-term conditions (LTCs) represents a huge opportunity to make a difference in reducing incidence and human burden as well as improving wider outcomes and reducing health and care costs for the NHS.
We know that people from marginalised communities (including Black, Asian and Minority Ethnic ‘BAME’ communities) are at a higher risk of developing these conditions . This has been further highlighted by a Public Health England report on Disparities in the risk and outcomes of COVID-19.
Working alongside the NHS, we co-produce and co-lead community led health and wellbeing events in response. After a successful pilot, we have delivered numerous community led health initiatives, identified further opportunities for early detection and run interventions across both physical and mental health conditions. We are now working to spread this approach across south west London and seek a community health co-ordinator to continue this work across the borough.
The role will include 1) growing and empowering our ‘Hub and Spoke’ network across south west London 2) delivering high quality health and wellbeing events in community settings and 3) measuring the impact and value alongside the WCEN team. You will be working with our locally trained health coaches, community leaders and NHS staff partners to reduce health inequalities in and across South West London.
We are looking for someone who has experience of project management, event organisation, physical health work, working with diverse communities and someone who can work occasional weekends.
35 hours per week (or 4 day weeks also available)
£26,500 per annum ( dependant on Experience – open for discussions)
